Request for the issuance of a Visa Debit EUR Card and/or for access to the e-Banking service and e-Statement service
1. I hereby confirm and acknowledge that I am bound by the General Terms Governing the Relationship between Customer-Bank [including but without limitation the Framework Contract, the Terms and Conditions of use of cards, the Terms and Conditions of use of the e-banking Service] (“the General Terms”), available at the banking centres and the Bank’s website www.eurobank.com.cy, and any other supplementary agreement and/or terms and conditions relating to the specific products/services which by the present I am applying for or for which I may apply from time to time, by the content of which I am bound, and the Bank’s Cut-off Times Table, the Bank’s Table of Commissions and Charges, the Bank's Card Charges, Commissions and Interest Table, and the U-Banking Special Commissions and Charges Table (if and to the extent applicable) (the “Commissions and Charges Tables”), and
2. Ι confirm that I have read and understood the content of the Privacy Notice, which forms an integral part of the General Terms, regarding the processing of my personal data for the purposes of entering into and carrying out a business and/or contractual relationship with the Bank and of my rights in this respect. I understand that the aforesaid document is available at the banking centres and the Bank’s website www.eurobank.com.cy. I also hereby undertake to deliver and/or make available the Privacy Notice to any other relevant person who is a natural person and whose personal data were disclosed herein above and/or might be disclosed in the future and/or notify them of the Privacy Notice, without undue delay.
3. I also understand and acknowledge that my application will be reviewed and/or assessed in accordance with the General Terms and that the Bank may reject the present application (including but not limited to any application for the issuance of cards) at its absolute discretion.
4. This application and the information and/or details given herein shall apply both for my existing personal accounts held with Eurobank Cyprus Ltd (the “Bank”) and for any account or accounts which shall be opened from time to time in my name by giving to the Bank instructions either orally and/or in writing and/or any other electronic means (“my personal Accounts”).
5. I further hereby affirm and confirm my previous authorizations and reiterate that I irrevocably authorize the Bank to charge and/or debit my Accounts with all transactions and fees resulting from the use of my Account(s) and/or the Bank’s services (including without limitation the e-Banking service) and/or the use of card(s) according to its effective from time to time, General Terms and the Commissions and Charges Tables without prior notice.
ISSUANCE OF A VISA DEBIT EUR CARD [Where applicable]
6. With the present application, I request the issuance of a Eurobank Visa Debit EUR Card in my name and accept and acknowledge that the Bank, without any prior notice, may increase/decrease the daily limit of my card and/or terminate the use of the card.
7. I hereby authorize the Bank to send SMS messages (SMS Alerts) to the mobile telephone number(s) held in the Bank’s records, for the transactions resulting from the use of the card(s). I undertake to immediately inform the Bank for any change in the said mobile telephone number(s) and understand that the said mobile telephone number(s) will also be used as contact number(s) in case of any suspicious transactions.
8. I also acknowledge that the debit card will be dispatched to my attention to the mailing address recorded and/or maintained in the Bank’s records with postal and/or courier service (depending on whether my declared mailing address is domestic or international), whereas my personal identification number (“PIN”) will be dispatched to my attention to the mailing address recorded and/or maintained in the Bank’s records with postal service. I further authorize the Bank to charge and/or debit any of my Accounts with the costs and/or expenses of such mailing and/or courier service.
ACCESS TO THE E-BANKING SERVICE AND E-STATEMENT SERVICE OF THE BANK [Where applicable]
9. With the present application, I request access to the e-Banking service of the Bank in order to perform transactions and retrieve information for all my personal Accounts held with the Bank. I further authorize the Bank to send me my e-banking User ID and/or password and/or the One Time Password (OTP), where applicable via e-mail to the e-mail address declared by me in this application form and/or through short message service (SMS) to the mobile phone number held in the Bank’s records.
10. I acknowledge that I shall be solely responsible for the safety and confidentiality of all and any personalized security credentials (User ID(s), password(s) and security device). I further acknowledge that the Bank may consider that the personalized security credentials have been duly received by me and I declare that the Bank shall bear no responsibility for any damage suffered by me caused by the failure of receipt of any such personalized security credentials due to wrong email address and/or mobile telephone number or due to technical problem or due to any other reason whatsoever, except if such reason is solely attributed to the Bank.
11. I also acknowledge that all of my personal Accounts with the Bank, including any accounts that may be opened in the future in my name, will be automatically connected to the Bank’s e-Banking service with full access (to retrieve information and create and authorize transactions). I understand that for any e-Banking access to any accounts opened or that will be opened in the future in my name jointly with any other person or persons at your Bank, I will need to sign relevant applications jointly with the other account holder(s) and submit them to the Bank for assessment.
12. I further acknowledge that the Bank, without any prior notice, may increase/decrease the daily limit of my e-Banking access to the Bank’s e-Banking service.
13. I additionally request and authorize the Bank to send me my account(s) statement(s) in relation to my personal Accounts by email monthly, until the Bank receives further notice on my end. I thus, request that the Bank proceeds to the dispatch of account(s) statement(s) relating to my personal Accounts by email and:
a. I acknowledge that the Bank will dispatch to me at the email address declared by me in this application form, the password that will be used for gaining access to the account statement(s).
b. I acknowledge that the dispatch via email by the Bank of the account statement(s) is conducted for my convenience. The Bank shall not be held responsible for any breach of any banking or other secrecy on the basis that the account statement(s) are being sent to the email address declared by me. I acknowledge that the Bank may consider that the account statement(s) for such accounts have been duly received by me. As a consequence, I declare that the Bank shall bear no responsibility for any damage I might incur due to not receiving any such account statement(s) or due to the provision to the Bank of wrong email address by me or due to a technical problem or due to any other reason whatsoever, not solely attributed to the Bank.
c. I shall not hold you responsible and/or dispute your actions which occurred based on the acts mentioned hereinabove and you proceeded in good faith.
d. I shall indemnify you and keep you indemnified for any demand, claim or action brought against you or for any expenses, losses, charges or damages, which you may incur as a result of your agreement to act as above.
14. Notwithstanding the above paragraphs in relation to the e-Statement service, I acknowledge and understand that in case I am an existing user of the e-Statement service of the Bank, the present application does not and/or will not in any way affect and/or amend any of my previous applications submitted to the Bank and that I will continue to receive my account(s) statement(s) in the frequency and manner previously requested, unless I otherwise inform the Bank by submitting a relevant application and/or request or unless I am otherwise informed by the Bank.
